SCHOOL PHYSICALS
THE SCHOOL PHYSICIAN WILL BE CONDUCTING SPRING SPORTS PHYSICALS ON TUESDAY, MARCH 6, 2012.

REQUIREMENTS FOR PARTICIPATION IN SPORTS 
Any student interested in participating in school athletics is REQUIRED to have a completed Health History Questionnaire (Part A) and an Annual Pre-Partcipation Physical Evaluation form (Part B) on file in the nurse's office.   It is recommended that your own physician conduct the Pre-Participation Evaluation. If you do not have a primary care physician/pediatrician, the school physician will be conducting exams prior to the start of each sports season. If your child needs a physical please have them come to the Nurse's Office to get the required paperwork so they will be able to tryout.
Please make sure that all Sports Physicals are current (within 365 days of tryout date).

ALL PAPERWORK MUST BE TURNED INTO THE NURSE'S OFFICE BY WEDNESDAY, MARCH 7, 2012.


ATHLETIC PHYSICALS
All students wishing to participate in athletics at Memorial Junior School must have a physical examination within 365 days (1 year) of the tryout date on file in the health office, and submit the additional necessary paperwork by the day tryouts begin in order to be eligible.
 
In an attempt to facilitate this process, determine below which category your child falls into and complete the appropriate paperwork. All forms are available in the health office or can be downloaded by clicking on the links below.
 
FIRST SPORT OF SCHOOL YEAR 
 
  1. Completed Annual Pre-Participation Physical Evaluation Forms are available here 
-         Part A: Health History Questionnaire - to be completed by parent/guardian and given to physician at time of physical examination
-         Part B: Physical Evaluation Form – to be completed by physician. 
 
  1. Permission Form (required for each sport) is available here.
-         To be completed and signed by the student and parent/guardian within 60 days of the start of each  sport tryout.
 
  1. Athletic Release Form (required annually)is available here.
-         To be completed and signed by parent/guardian 
 

 

SECOND/THIRD SPORT OF SCHOOL YEAR 
(ATHLETIC PACKET – UPDATE)
 
1.      Part C: Health History Update – to be completed and signed by parent/guardian  (Click on ATHLETIC PACKET line above)
 
2.      Permission Form (required for each sport)

-         To be completed and signed by the student and parent/guardian within 60 days of the start of each sport tryout.
 
Reminder – All forms must be submitted directly to the health office and approved by the School Doctor before an athlete can tryout.  To ensure that your child can tryout on time, please adhere to the deadlines established before the start of each season.

MEDICATIONS
All medications, including over-the-counter, must be kept in the nurse's office to be administered only by the nurse.  A doctor's note with parental permission must be provided in order to administer medication (appropriate forms are available in the health office or can be downloaded by clicking on the links below). 

Student  self-administration of medication for potentially life-threatening illnesses, such as asthma or anaphylaxic reaction, is permitted and must also be arranged with the school nurse.

NO CHILD IS PERMITTED TO CARRY MEDICATIONS (Exceptions include self-administration of epi-pens and inhalers).
